Goal is to make a simple web application for an eye doctor containing about 10 php pages and 10 tables.
Pages contains: list of patients, details of patient, pages to input consultation, prescription of glasses and medications, and facturation. There are also pages to display list of all consultations, prescription and facturations for 1 patient.
Web application will be made using MySQL and PHP 5.0 with CSS and must be W3C valid.
Ajax will be used to ease completion of fields or selection in a list. Two little scripts will be homemade to fill easily some simple parameters fields.
Specifications are given in attached files: page layout, nomenclature of each field, overview of database table relations.
The main thinking part is already done, most part left is coding. Estimated time to code this web application is about 5 to 6 days maximum.
Lines of code must be documented as possible, database architecture must be given with the code.
Dear Sir, I understand ur web requirment, also you have to provide the 10 table with field,(or some kind of questioneres(because we are not having medical consultant), forums you fill at the clinic/or with your patient).
